SMIC Application Form - This application is for tenancy at the Southwest Michigan Innovation Center as a physical location.

SMIC Affiliate Application Form - This application is for affiliate status; meaning that your company will enjoy all of the benefits of incubation without having a physical location here at the Innovation Center. This includes access to various trainings and seminars, and the review process with our management team.
